All fields below must be completed. Please submit only one issue at a time below. If needed, you may submit multiple separate issues by returning to this form as needed and proposing modifications separately. If more than one bylaw is affected in the same issue, please insert each bylaw into its separate line below. Without consideration to the intended proposal, we may decline or modify proposals that do not meet formatting or other administrative requirements.
